For Telehealth or In-Person Appointments, please login to the patient portal to request an appointment or call your doctor's office.
Any provider listed on the Patient Portal who does not offer direct appointment booking, will display with the option to request an appointment.
Please contact your insurance plan to verify coverage and benefit requirements before booking your appointment.
Login to your Patient Portal account, go to the Schedule tab and choose Request Appointment.
Direct booking of appointments is best done using a desktop device and not a mobile device. If this is a medical emergency, please call 911 or go to the nearest emergency room. Please call your provider’s office ASAP if you have to Cancel or Reschedule an appointment.
