Must Have  A Core Provider
The Core lab accepts all patients who have a Core provider. You must have a Core primary care physician if you have a lab order from a non-Core specialist.

Lab Work Sent Outside of Core
You may receive a separate bill for any lab work sent outside of Core for processing. You may also be subject to paying an office co-pay for your laboratory services. Please contact your health plan with any questions regarding your coverage.

Anthem Site of Service
For patients with the Anthem Site of Service benefit design, it is important to share with you that cost shares will be waived when a Core lab is chosen.  Copays, coinsurance and deductibles will be waived for all Anthem Site of Service members when using a Core lab.

If fasting is required, please do not eat or drink for 10 – 12 hours before your lab test, with the exception of water.  Drinking water and staying hydrated will improve your experience.  Please take medications as directed by your provider.

Lab Results - upon your request
Most lab test results are available to request on the Patient Portal within 5 days after the test is performed. To view your most recent test results, a new PHR must be requested. How to Request Your Personal Health Record (PHR)
