Congestive Heart Failure (CHF) Clinic

The clinic starts with two afternoon appointments per week. Patients are enrolled upon discharge from the hospital or at the request of their Primary Care Provider.  Patients referred to the CHF Clinic will be seen in one week or less from discharge.  Patients will be contacted, after being referred to the clinic, to assure no decompensation has taken place since discharge.   Our staff consists of a physician, nurse practitioner and a licensed nurse who work collaboratively to provide our patients with the education and treatment to help keep them healthy and out of the hospital.
